I. Tổng quan tính thấm ruột và nơ ron ruột tá tràng ở chuột tự kỷ

Hội chứng tự kỷ (ASD) là rối loạn phát triển thần kinh phổ biến. Bệnh nhân tự kỷ thường gặp các vấn đề tiêu hóa mạn tính. Táo bón, tiêu chảy và đau bụng xuất hiện với tần suất cao. Trục não-ruột tạo ra mối liên hệ hai chiều giữa hệ thần kinh trung ương và đường tiêu hóa. Luận văn thạc sĩ khoa học của Joshua Kenneth Williams tại Đại học RMIT tập trung vào tính thấm ruột và đặc tính nơ-ron ruột tá tràng. 1.1. Rối loạn tiêu hóa và trục não ruột trong hội chứng tự kỷ

Trục não-ruột là mạng lưới giao tiếp phức tạp giữa não bộ và đường tiêu hóa. Mạng lưới này vận hành qua hệ thần kinh, hệ miễn dịch và nội tiết. Bệnh nhân tự kỷ thường biểu hiện các triệu chứng đường ruột nghiêm trọng. Tình trạng viêm ruột cục bộ làm thay đổi hành vi và cảm xúc của người bệnh. Các phân tử truyền tin từ ruột có thể vượt qua hàng rào máu não. Hệ thần kinh ruột chứa hàng triệu tế bào thần kinh tự chủ. Hệ thống này kiểm soát trực tiếp các phản xạ co bóp và bài tiết của ống tiêu hóa. Khi tín hiệu thần kinh ruột bị rối loạn, chức năng tiêu hóa suy giảm nghiêm trọng. Mối liên hệ chặt chẽ này thúc đẩy các nhà khoa học tìm kiếm cơ chế phân tử gốc rễ.

1.2. Mô hình chuột chuyển gen mang đột biến Neuroligin 3 R451C

Gen Neuroligin-3 mã hóa protein bám dính synap tại hệ thần kinh. Đột biến điểm R451C làm thay đổi chức năng kết nối synap thần kinh trung ương. Mô hình chuột mang đột biến NL3R451C thể hiện các hành vi điển hình của chứng tự kỷ. Chuột giảm giao tiếp xã hội và tăng tính tương tác lặp lại. Đột biến này cũng gây ra những thay đổi rõ rệt trong hệ thần kinh ruột. Các nhà khoa học ghi nhận sự gia tăng số lượng nơ-ron tại hỗng tràng và manh tràng. Chuột đột biến còn có sự thay đổi về độ nhạy cảm thụ thể GABA và nhu động ruột. Sử dụng mô hình chuột này giúp giải mã chi tiết các bất thường tại tá tràng.

II. Phân tích tính thấm ruột và protein liên kết ở chuột tự kỷ

Hàng rào biểu mô ruột bảo vệ cơ thể khỏi vi khuẩn và độc tố ngoại sinh. Cấu trúc này bao gồm lớp tế bào biểu mô và các phức hợp liên kết khe (tight junction). Sự toàn vẹn của liên kết khe duy trì tính thấm chọn lọc của ruột non. Ở bệnh nhân và mô hình động vật tự kỷ, biểu hiện gen mã hóa protein liên kết bị suy giảm rõ rệt. Hiện tượng này dẫn đến tình trạng tăng tính thấm ruột hoặc hội chứng rò rỉ ruột. Độc tố vi khuẩn và kháng nguyên lạ dễ dàng xâm nhập vào tuần hoàn máu. Phản ứng viêm toàn thân kích hoạt và gây tổn thương hàng rào máu não. Nghiên cứu sinh học phân tử chỉ ra sự biến đổi của các protein claudin-1, claudin-5, occludin và tricellulin. Sự mất cân bằng này làm lỏng lẻo khoảng gian bào biểu mô tá tràng. Hậu quả là chức năng hấp thu dưỡng chất bị suy thoái nghiêm trọng. Việc phân tích biến đổi tính thấm theo từng vùng ruột giúp xác định tổn thương cục bộ chính xác.

2.1. Cơ chế phân tử của suy giảm protein liên kết biểu mô ruột

Protein liên kết khe duy trì rào cản vật lý giữa các tế bào biểu mô ruột. Các phân tử occludin, tricellulin và claudin-1 đóng vai trò then chốt trong cấu trúc này. Khi biểu hiện của các gen này suy giảm, liên kết biểu mô trở nên lỏng lẻo. Ngược lại, mức độ biểu hiện của claudin-2 và claudin-15 lại có xu hướng tăng cao. Sự mất cân bằng giữa các nhóm protein tạo ra các lỗ rò rỉ chọn lọc ion và phân tử lớn. Các mẫu mô phân tích từ mô hình chuột tự kỷ cho thấy sự phá vỡ cấu trúc liên kết biểu mô. Tế bào ruột mất khả năng kiểm soát sự khuếch tán qua khoảng gian bào. Quá trình viêm biểu mô tiếp tục kích thích chu trình phá hủy tế bào.

2.2. Sự sai khác về tính thấm giữa các vùng ruột non và đại tràng

Tính thấm của ống tiêu hóa có sự phân hóa rõ rệt tùy theo từng đoạn giải phẫu. Tá tràng và hỗng tràng chịu trách nhiệm tiêu hóa chính nên có diện tích tiếp xúc lớn. Tại manh tràng và đại tràng của chuột NL3R451C, các biến đổi nơ-ron diễn ra không đồng nhất. Nghiên cứu thực nghiệm cho thấy đại tràng vẫn duy trì nhu động cơ bản trong điều kiện bình thường. Tuy nhiên, tính thấm biểu mô ở đoạn ruột non lại suy giảm tính chọn lọc nhanh chóng. Sự rò rỉ phân tử xảy ra mạnh mẽ tại tá tràng trước khi lan rộng sang các vùng khác. III. Phương pháp đo điện sinh lý nơ ron ruột tá tràng ở chuột tự kỷ

Nghiên cứu ứng dụng tổ hợp phương pháp điện sinh lý học và mô học tiên tiến. Kỹ thuật ghi điện thế nội bào (intracellular electrophysiology) được dùng để phân tích nơ-ron ruột tá tràng. Phương pháp này ghi lại các đặc tính điện sinh lý cơ bản của màng tế bào thần kinh. Điện thế nghỉ, ngưỡng kích thích và biên độ điện thế hoạt động được đo đạc chuẩn xác. Nhuộm hóa mô miễn dịch (immunocytochemistry) hỗ trợ định danh các phân nhóm nơ-ron đặc hiệu. Hai mạng lưới chính là đám rối dưới niêm mạc (submucosal plexus) và đám rối cơ ruột (myenteric plexus) được bóc tách cẩn thận. Mức độ biểu hiện của enzyme tổng hợp oxit nitric (NOS) và thụ thể GABA_A được định lượng chi tiết. Thí nghiệm bổ sung các chất đối kháng như gabazine và bicuculline để khảo sát thụ thể GABA_A. Phương pháp buồng Ussing hoặc đo huỳnh quang FITC-dextran được tiến hành để xác định tính thấm biểu mô tá tràng. Quy trình thí nghiệm đảm bảo tính khách quan và độ chính xác khoa học cao.

3.1. Kỹ thuật ghi điện sinh lý nội bào và hóa mô miễn dịch ENS

Kỹ thuật vi điện cực thủy tinh được cắm trực tiếp vào thân nơ-ron ruột. Thiết bị ghi nhận sự thay đổi điện thế màng với độ nhạy milivolt. Tín hiệu xung thần kinh được số hóa và phân tích tự động qua phần mềm chuyên dụng. Song song đó, kỹ thuật hóa mô miễn dịch nhuộm huỳnh quang định vị các dấu ấn thần kinh. Kháng thể kháng Hu, nNOS và choline acetyltransferase làm sáng tỏ cấu trúc quần thể nơ-ron. Phương pháp này cho phép đếm số lượng nơ-ron trong từng hạch thần kinh ruột tá tràng. Sự kết hợp giữa chức năng điện học và hình thái mô học cung cấp cái nhìn toàn diện về tổn thương thần kinh ruột.

3.2. Đánh giá tính nhạy cảm thụ thể GABA A và dẫn truyền thần kinh

Thụ thể GABA_A đóng vai trò điều biến quan trọng trong hệ thống thần kinh ruột. Các thử nghiệm co thắt cơ trơn sử dụng thuốc chẹn thụ thể GABA_A nhằm kiểm tra phản ứng nhu động. Khi ức chế thụ thể bằng bicuculline hoặc gabazine, mô ruột chuột đột biến bộc lộ sự suy giảm vận động bất thường. Kết quả phản ánh sự gia tăng độ nhạy cảm của thụ thể GABA_A tại đường tiêu hóa. Sự biến đổi này tương đồng với các sai hỏng dẫn truyền GABAergic từng ghi nhận trên não chuột tự kỷ. Thí nghiệm chứng minh đột biến Neuroligin-3 gây tác động đồng thời lên cả hệ thần kinh trung ương và ngoại biên.

IV. Kết luận về tính thấm ruột và hướng can thiệp hội chứng tự kỷ

Luận văn thạc sĩ khẳng định đột biến NL3R451C làm thay đổi cấu trúc và chức năng nơ-ron tá tràng. Hàng rào biểu mô ruột bị suy giảm tính toàn vẹn tạo điều kiện cho độc tố thẩm thấu. Sự thay đổi mật độ nơ-ron và thụ thể GABA_A tại tá tràng giải thích nguyên nhân gây rối loạn nhu động tiêu hóa. Đây là bằng chứng thực nghiệm quan trọng củng cố học thuyết trục não-ruột trong bệnh sinh tự kỷ. Phát hiện này mở ra nhiều triển vọng mới cho y học can thiệp. Các liệu pháp phục hồi liên kết biểu mô ruột có thể làm giảm tải lượng viêm hệ thống. Đồng thời, việc điều biến hoạt động điện của hệ thần kinh ruột hứa hẹn cải thiện cả triệu chứng tiêu hóa lẫn hành vi tâm thần. Thuốc củng cố liên kết protein biểu mô giúp ngăn chặn độc tố xâm nhập vào máu. Ngoài ra, việc bổ sung các chủng vi sinh probiotics hỗ trợ phục hồi tính toàn vẹn của niêm mạc. In addition to these behavioural characteristics, as many as ninety percent of individuals with ASD exhibit gastrointestinal (GI) issues.

A missense mutation at position R451C in the synaptic adhesion protein, neuroligin-3 (NLGN3) was previously identified in patients with ASD. The Nlgn3 gene is present in both the brain and the intrinsic neural network of the gut, the enteric nervous system (ENS). Within the ENS, the submucosal plexus (SMP) regulates mucosal barrier functions such as secretion and permeability, while the myenteric plexus predominantly regulates gut motility. Clinical studies have shown that individuals with ASD who experience GI disturbances have elevated blood concentrations of simple sugars such as mannitol and lactulose, which is a marker for increased intestinal permeability.

Previous data from the current laboratory showed faster small intestinal transit time in NL3R451C mice compared to wild-type littermates. We hypothesise that the R451C mutation affects intestinal function (i., permeability and motility) in mice. Duodenal, jejunal, ileal and colonic paracellular permeability was assessed using an ex-vivo whole tissue assay in non-fasted and fasted mice. The effects of the R451C mutation on functional characteristics of duodenal myenteric neurons were also investigated in wild type mice.

No significant differences in intestinal permeability were observed between non-fasted NL3R451C mice and wild-type littermates in any intestinal region. In a fasted state, however, NL3R451C mice showed increased intestinal permeability in the duodenum, jejunum, ileum and colon when compared to wild-type littermates. The addition of L-glutamine and caffeine rescued the increased intestinal permeability in the duodenum, jejunum, ileum and colon in NL3R451C mice. To characterise functional neuronal properties in the ENS, action potentials and baseline cellular parameters were recorded from myenteric neurons using the whole-cell patch clamping technique on Longitudinal Muscle Myenteric Plexus Preparations (LMMPs).

The probability of obtaining a 1 recording was greater in younger (2-4-week-old) Swiss white and C57BL/6 mice in comparison to older (6-12-week-old) C57Bl/6 mice. Different action potential firing patterns were observed which may correlate with different functional subgroups of myenteric neurons. Characterising intestinal permeability and categorizing enteric neurons by their functional profiles may be useful for understanding and reducing GI symptoms, chronic low-grade systemic inflammation, and core neurological symptoms in people with ASD.0 Autism Spectrum Disorder (ASD) overview ASD is a complex neurodevelopmental disorder that results in a range of clinical traits that can differ in type and severity in each individual (Vahia, 2013). For instance, an individual with ASD will often display persistent deficits in social- emotional reciprocity, nonverbal communicative behaviours (hand gestures and eye contact), verbal communication and developing, maintaining and understanding relationships. In addition, the individual must have persistent deficits in at least two types of repetitive or restricted behaviour (Vahia, 2013). These include motor movements that are repetitive or stereotyped, difficulties in managing distress at small changes, fixating on interests and hypo- or hyperactivity (Vahia, 2013).
