I. Tổng quan về thiết kế tô pô giàn hợp kim titan xốp

Vật liệu cấy ghép chỉnh hình truyền thống bằng hợp kim titan đặc thường có độ cứng quá cao so với xương người. Kim loại titan nguyên chất và hợp kim Ti6Al4V sở hữu mô đun đàn hồi khoảng 110 đến 115 GPa. Trong khi đó, xương vỏ của con người chỉ có mô đun đàn hồi từ 10 đến 30 GPa. Sự chênh lệch cơ học này gây ra hiện tượng che chắn ứng suất nguy hiểm. Hiện tượng này dẫn đến thoái hóa xương và làm lỏng khớp cấy ghép sau phẫu thuật. Nghiên cứu luận văn thạc sĩ kỹ thuật tập trung giải quyết triệt để vấn đề trên. Giải pháp cốt lõi là ứng dụng thiết kế tô pô để tạo ra các cấu trúc giàn không gian xốp. Cấu trúc xốp giúp điều chỉnh độ cứng cơ học của vật liệu về mức tối ưu. Đồng thời, các lỗ rỗng liên thông tạo không gian lý tưởng cho tế bào xương phát triển. 1.1. Vai trò của hợp kim titan trong chỉnh hình

Titan và các hợp kim titan như Ti6Al4V là vật liệu tiêu chuẩn trong cấy ghép y khoa. Vật liệu này có độ bền riêng rất cao và khả năng chống ăn mòn tuyệt vời. Màng oxit thụ động bề mặt bảo vệ kim loại khỏi sự tấn công của dịch cơ thể. Nhờ đó, titan đạt độ tương thích sinh học cao và không gây độc hại cho tế bào mô. Bệnh nhân tiếp nhận mảnh ghép titan ít gặp phản ứng đào thải miễn dịch. Tuy nhiên, dạng cấu trúc kim loại đặc truyền thống vẫn bộc lộ hạn chế lớn về cơ học. Việc chuyển đổi từ titan dạng khối sang dạng xốp là xu hướng tất yếu của y học hiện đại.

1.2. Mục tiêu nghiên cứu của luận văn thạc sĩ

Luận văn thạc sĩ kỹ thuật của tác giả Li Yuan tập trung phát triển các cấu trúc giàn titan xốp tối ưu. Đề tài áp dụng các nguyên lý hình học bề mặt tối thiểu tuần hoàn ba chiều Gyroid. Mục tiêu là tạo ra cấu trúc cấy ghép có mô đun đàn hồi tiệm cận xương tự nhiên. Đồng thời, cấu trúc phải đảm bảo độ bền chịu lực tĩnh và độ bền mỏi vượt trội. Nghiên cứu kết hợp mô phỏng phần tử hữu hạn với công nghệ in 3D nung chảy laser chọn lọc. Kết quả cung cấp giải pháp toàn diện từ thiết kế hình học đến quy trình chế tạo thực nghiệm.

II. Phân tích hiện tượng che chắn ứng suất ở titan xốp y sinh

Sự không tương thích cơ học giữa mô cấy và xương là rào cản lâm sàng lớn nhất. Titan đặc có độ cứng gấp gần mười lần xương vỏ người. Khi chịu tải trọng vận động, phần kim loại cứng sẽ gánh hầu hết lực tác động. Xương xung quanh không nhận đủ kích thích cơ học cần thiết. Hiện tượng này dẫn đến tình trạng tiêu xương cục bộ theo định luật Wolff. Hệ quả là mảnh cấy ghép bị nới lỏng theo thời gian và gây đau đớn cho người bệnh. Ngoài ra, cấu trúc titan khối không cho phép mô xương mọc sâu vào bên trong. Sự liên kết giữa bề mặt nhẵn và xương chủ yếu là bám dính cơ học yếu ớt. Bên cạnh đó, quá trình chế tạo bồi đắp kim loại như SLM cũng gặp thách thức về chất lượng bề mặt. Các hạt bột titan nóng chảy dở dang thường bám dính trên thành vách cấu trúc xốp. Lớp hạt này làm tăng độ nhám bề mặt nhưng lại giảm mạnh độ bền mỏi của vật liệu cấy ghép. Những vết nứt tế vi dễ phát sinh từ các điểm khuyết tật này dưới tải trọng tuần hoàn.

2.1. Tác động của hiệu ứng che chắn ứng suất

Hiệu ứng che chắn ứng suất xuất hiện khi mô đun đàn hồi của kim loại cấy ghép vượt trội so với xương. Xương tự nhiên là mô sống thích nghi liên tục với ứng suất cơ học tác động. Khi mô cấy kim loại gánh toàn bộ tải trọng, xương bị thiếu kích thích cơ học. Tế bào hủy xương sẽ hoạt động mạnh hơn tế bào tạo xương. Hiện tượng tiêu xương diễn ra âm thầm xung quanh khớp nhân tạo. Quá trình này dẫn đến mất xương nghiêm trọng và làm lỏng liên kết ghép. Bệnh nhân thường phải trải qua phẫu thuật thay lại khớp đầy rủi ro và tốn kém.

2.2. Nhược điểm bề mặt và độ bền mỏi khi in 3D

Quá trình in 3D laser kim loại SLM tạo ra bề mặt có độ nhám tương đối cao. Nguyên nhân bắt nguồn từ các hạt bột kim loại dính bám chưa nóng chảy hoàn toàn. Độ nhám cao có thể kích thích tế bào bám dính ban đầu. Tuy nhiên, các rãnh khuyết tật bề mặt lại là nguồn gốc tập trung ứng suất cơ học. Dưới tải trọng mỏi chu kỳ lớn khi vận động, các vết nứt tế vi dễ dàng khởi sinh. Điều này làm giảm đáng kể tuổi thọ làm việc của giàn titan xốp. Do đó, quy trình xử lý sau gia công bằng nhiệt luyện và phun cát là bắt buộc để khử ứng suất dư.

III. Giải pháp thiết kế tô pô TPMS Gyroid và in 3D SLM

Thiết kế cấu trúc bề mặt tối thiểu tuần hoàn ba chiều Gyroid đại diện cho giải pháp tối ưu hình học hiện đại. Cấu trúc TPMS Gyroid có độ cong trung bình tại mọi điểm bằng không và phân bố vách trơn nhẵn liên tục. Thiết kế này loại bỏ các điểm nối sắc nhọn thường thấy ở mạng tinh thể dạng thanh dầm truyền thống. Nhờ đó, vật liệu giảm thiểu tối đa hiện tượng tập trung ứng suất khi chịu tải nặng. Độ xốp và kích thước lỗ rỗng có thể tùy biến chính xác theo hàm toán học định sẵn. Mô hình hóa phần tử hữu hạn đóng vai trò quan trọng trong việc dự đoán đặc tính cơ lý. Các thông số như mô đun đàn hồi Young, độ bền uốn và phân bố ứng suất von Mises được tính toán chi tiết. Công nghệ nung chảy laser chọn lọc SLM được ứng dụng để hiện thực hóa các mẫu cấy ghép phức tạp từ bột Ti6Al4V. Buồng in khí trơ Argon ngăn ngừa hiện tượng oxy hóa kim loại trong suốt quá trình nung chảy. Sản phẩm tạo ra có độ xốp kiểm soát từ 60% đến 80%, hoàn toàn khớp với cơ tính của xương người.

3.1. Mô hình hóa bề mặt tối thiểu TPMS Gyroid

Cấu trúc Gyroid thuộc nhóm bề mặt tối thiểu tuần hoàn ba chiều với phương trình toán học lượng giác phức tạp. Cấu trúc này phân chia không gian thành hai miền liên tục đan xen nhau mà không tự giao cắt. Thiết kế tạo ra độ thấm hút thủy lực xuất sắc cho dòng chất lưu sinh học lưu thông. Tế bào xương dễ dàng tiếp cận dưỡng chất và oxy qua các kênh dẫn mở. Bề mặt cong mượt mà giúp phân tán đều ứng suất dưới tải trọng nén và xoắn. Điều này giúp nâng cao đáng kể độ bền mỏi so với cấu trúc tổ ong hoặc khối lập phương truyền thống.

3.2. Chế tạo bồi đắp bằng công nghệ nung chảy laser SLM

Công nghệ in 3D kim loại SLM sử dụng chùm tia laser sợi quang công suất cao để nung chảy bột hợp kim titan. Tia laser quét chính xác từng lớp bột mỏng có độ dày khoảng 30 đến 60 micromet theo thiết kế CAD. Quy trình được thực hiện nghiêm ngặt trong môi trường khí bảo vệ Argon hoặc Nitơ. Nhiệt độ gia nhiệt sơ bộ bàn in duy trì từ 100 đến 200 độ C nhằm giảm sốc nhiệt. Phương pháp cho phép chế tạo hình học xốp có độ chính xác kích thước cao. Các bước nhiệt luyện và xử lý bề mặt tiếp theo giúp tối ưu hóa cơ tính cấu trúc trước khi cấy ghép.

IV. Kết luận và ứng dụng giàn titan xốp trong chỉnh hình

Nghiên cứu thiết kế tô pô giàn hợp kim titan xốp khẳng định bước đột phá quan trọng trong kỹ thuật cấy ghép chỉnh hình. Sự kết hợp giữa cấu trúc toán học TPMS Gyroid và công nghệ in 3D SLM giải quyết trọn vẹn sự mất cân bằng cơ học. Mô đun đàn hồi của mảnh cấy ghép titan xốp có thể giảm xuống ngưỡng 5 đến 25 GPa. Con số này tương thích hoàn hảo với đặc tính cơ học của xương tự nhiên. Hiện tượng che chắn ứng suất và tiêu xương được triệt tiêu hiệu quả. Ứng dụng lâm sàng của giải pháp này mở rộng từ thay khớp háng, khớp gối nhân tạo đến phục hồi khiếm khuyết xương hàm mặt. Không gian lỗ rỗng liên thông thúc đẩy mạch máu và tế bào xương bám dính sâu vào lõi cấy ghép. Sự khóa cơ sinh học vững chắc giúp giảm thiểu tối đa nguy cơ phẫu thuật thay lại. Hơn nữa, vật liệu kim loại phân hủy sinh học như magie cũng có thể ứng dụng mô hình thiết kế này cho cấy ghép tạm thời. Hướng đi này định hình tương lai của ngành y học cấy ghép cá nhân hóa.

4.1. Tiềm năng ứng dụng lâm sàng trong chỉnh hình

Cấu trúc giàn titan xốp TPMS mang lại giải pháp cấy ghép hoàn hảo cho các ca phẫu thuật phức tạp. Sản phẩm có thể ứng dụng trong chuôi khớp háng nhân tạo, tấm nẹp cố định cột sống hoặc khung tái tạo hộp sọ. Khả năng tùy biến độ xốp theo giải phẫu từng bệnh nhân giúp tối ưu độ khít cơ học. Sự tích hợp mô xương nhanh chóng rút ngắn đáng kể thời gian hồi phục chức năng vận động. Tỷ lệ biến chứng và nguy cơ đào thải mảnh ghép giảm rõ rệt. Đây là nền tảng vững chắc để chuyển giao công nghệ vào thực tiễn y khoa.

4.2. Triển vọng phát triển vật liệu cấy ghép tương lai

Tương lai của ngành cấy ghép y sinh hướng đến việc tích hợp vật liệu phân hủy sinh học và in 4D. Các kim loại như magie và kẽm xốp có thể tự tiêu biến sau khi mô xương lành hẳn. Thiết kế tô pô TPMS kết hợp vật liệu phân hủy sinh học sẽ loại bỏ hoàn toàn nhu cầu phẫu thuật tháo bỏ nẹp vít. Đồng thời, kỹ thuật biến đổi độ xốp chức năng gradient cho phép tạo cấu trúc phỏng sinh học đa tầng hoàn chỉnh. Nghiên cứu của Li Yuan đặt viên gạch nền móng quan trọng cho các thế hệ vật liệu thông minh tiếp theo.

These new materials showed excellent performances in the load-bearing implant applications and can be used as a replacement of bones and joints in clinical surgeries. Nowadays, with the assistance of additive manufacturing technology, medical implants with complicated topological structures and shapes which were previously not manufacturable with conventional manufacturing approaches are designed and fabricated. It not only significantly increases the feasibility of bone joint replacement by biomedical materials but also has dramatic influences on the development of modern medicine.

However, the design of new implants is a challenging task in order to ensure the high quality of implants to avert both biocompatible and mechanical risks. Firstly, when designing the implants, the properties of the biomaterials should be considered, including the physical properties (e. density, elastic modulus) and the chemical properties (e. Secondly, the potential negative effects on bone defect treatment should be examined to avoid issues caused by any inappropriate mechanical behaviour of materials.

Finally, and more importantly, due to the mismatch of elastic modulus between natural bone and the implants, stress shielding which is another major mechanical issue of metallic implants has to be minimized. A large mismatch of elastic modulus may lead to bone atrophy and implant loosening. 13 A new porous titanium scaffold which exhibits an architecture mimicking that of human bones was developed in this research for the application of implants. The scaffold will be constructed by multiple unit cell sizes, which enables adjusting the elastic modulus to meet different requirements.

The porous structure can provide the necessary support for cells to proliferate and maintain their differentiated functions, and their structure defines the ultimate shape of the new bone created during growth processes. A comprehensive literature review was conducted by summarizing the state-of-the-art technologies and critical issues associated with the results of the biomedical porous material fabricated with different methods. Different triply periodic minimal surface (TPMS) structures and their characteristics were investigated, especially for gyroid structures. Three-dimensional modelling of gyroid structure (unit cell and small scaffold) were conducted for further analysis of the design.

To address the issue of the higher elastic modulus of Ti, a 3mm Network-based gyroid unit cell and small scaled scaffolds previous designed were analysed in regard to the mechanical behaviours. Specifically, the models were optimized for evaluating the Young’ modules, in order to meet the certain requirement. In addition, a 3mm cubic open-cellular structure was developed as the comparison group in this simulation. Keywords: Metal implant design; Additive manufacturing; Porosity; Stress shielding; TPMS structures; Gyroid 14 Chapter 1 Introduction 1.1 Background Bone is an anisotropic connective tissue consisting of hydroxyapatite, collagen and water.

It provides frames for skeleton structural stability, protects vital organs and specialized tissues, and supports the mechanical actions of soft tissues. Bones have excellent regenerative properties and self-healing abilities for the body to recover from physical injury. However, bone's regenerative ability becomes weaker when people become older; and if suffered from a serious trauma or a systemic bone disease, bones may be found extremely difficult to recover their self-healing functions. The needs for orthopaedic implants have dramatically increased in the last two decades.

Patients are expecting treatments that allow them to maintain their daily activities and quality of life. Taking Knee replacement as an example (Figure 1.1), the rates of total knee replacement (TKR) have been significantly increased from 1991 to 2006 [1]. In the US, patients spent over US$9 billion on the total knee arthroplasty (TKA) in 2009 and the major demand of TKA was in group aged 45–64 years [2]. The value of the biomaterials market was US$94.1 billion in 2012, and increased to US$134.3 billion in 2017 worldwide [3].

This rapid increase in the biomaterials market has to a certain extent brought benefits for the development of bone tissue engineering (BTE).1 Orthopedic implants in a knee replacement surgery [4] Advanced biomaterials, fabrication methods, and the structural designs of medical devices have been greatly improved in the last twenty years. Materials for medical applications need to meet several criteria, and designed implants should morphologically mimic bone structure and support bone tissue formation (osteogenesis). Biocompatibility, mechanical properties, and biodegradability are the fundamental elements that must be considered. The structure of bone is almost completely constituted by hydroxyapatite crystal, a mineral form of calcium apatite, within an organic matrix of collagen [4].

Of this collagen, 95% is type I, providing the structural integrity for connective tissues in bones, tendons, and ligaments. The remaining 5% of the bone is a combination of proteoglycans and numerous non-collagenous proteins. An effective implant will be accepted by the human body and function properly. An inferior orthopaedic device can trigger serious issues in patients.

In Australia, the Therapeutic Goods Administration (TGA) provides 16 regulations for medical devices, such as Australian regulatory guidelines for medical devices (ARGMD), which provide guidance to assist the manufacturers and sponsors of medical devices in meeting the regulatory requirements for legally supplying a medical device in Australia [5,6]. Metal-AM fabrication has been extensively explored in the last decade and this technology has been successfully used in the biomedical field. 3D printed physical models can provide a detailed visualization of clinic cases before surgery, which is a practical approach for surgery planning and accurate diagnosis. Surgeons and medical doctors can simulate surgery processes on 3D printed models to examine the outcomes of the surgery and find any potential surgical risk and failing factors [29, 30].

More importantly, AM technology can produce customized implants for bone replacement and fixation. Since the current implant structure design becomes increasingly complex to meet different requirements (e., interconnected porous TPMS scaffold), it could be challenging to manufacture such a metallic implant via conventional fabrication methods such as casting, forging, milling, and turning. This technology is also capable of fabricating complex porous structures with both micro and macro porosity. Due to the high solidification rate of AM techniques, AM-produced implants may exhibit high strength [31].
