I. Giới Thiệu Nhu Cầu Chăm Sóc Sức Khỏe Mẹ Tại Việt Nam

Nhu cầu chăm sóc sức khỏe mẹ là một vấn đề cấp thiết trên toàn cầu, đặc biệt ở các quốc gia có thu nhập thấp. Tổ chức Y tế Thế giới (WHO) đã báo cáo về tỷ lệ tử vong mẹ (MMR) trên toàn cầu vào năm 2013, tuy nhiên, tỷ lệ này ở các khu vực đang phát triển cao hơn 14 lần so với các khu vực phát triển. Mặc dù tỷ lệ tử vong mẹ đang giảm trên toàn thế giới, nhưng vẫn chưa đạt được mục tiêu của Mục tiêu Phát triển Thiên niên kỷ số 5 (MDG 5). Việt Nam cũng không nằm ngoài xu hướng này. Bài viết này sẽ tập trung phân tích các yếu tố ảnh hưởng đến nhu cầu chăm sóc thai kỳlựa chọn địa điểm sinh nở tại Việt Nam.

1.1. Tầm quan trọng của chăm sóc sức khỏe sinh sản và chăm sóc thai kỳ

Chăm sóc sức khỏe sinh sản và đặc biệt là chăm sóc thai kỳ đóng vai trò quan trọng trong việc giảm tỷ lệ tử vong mẹ và trẻ sơ sinh. Khám thai định kỳ giúp phát hiện sớm các nguy cơ và biến chứng có thể xảy ra trong quá trình mang thai và sinh nở. Việc chăm sóc sức khỏe trước sinh đúng cách còn giúp cải thiện sức khỏe của bà mẹ và sự phát triển của thai nhi, từ đó giảm thiểu các vấn đề sức khỏe sau này. Theo WHO, việc sinh con tại các cơ sở y tế, có sự hỗ trợ của nhân viên y tế có kỹ năng là vô cùng quan trọng.

1.2. Thực trạng tỷ lệ tử vong mẹ và tiếp cận dịch vụ y tế ở Việt Nam

Việt Nam đã có những tiến bộ đáng kể trong việc cải thiện sức khỏe bà mẹ, thể hiện qua việc giảm tỷ lệ tử vong mẹ. Tuy nhiên, vẫn còn tồn tại sự chênh lệch lớn giữa các vùng miền, các nhóm dân tộc và giữa thành thị và nông thôn trong việc tiếp cận các dịch vụ y tế. Các khu vực miền núi thường có tỷ lệ tử vong mẹ cao hơn gấp ba lần so với các khu vực đồng bằng, và các nhóm dân tộc thiểu số gặp nhiều khó khăn hơn trong việc tiếp cận các dịch vụ chăm sóc sức khỏe thai kỳ chất lượng.

II. Thách Thức Tiếp Cận Dịch Vụ Chăm Sóc Thai Kỳ tại Việt Nam

Mặc dù tỷ lệ phụ nữ mang thai được khám thai ít nhất một lần là khá cao, nhưng số lần khám thai và chất lượng dịch vụ chăm sóc vẫn còn là một vấn đề. Nhiều nghiên cứu chỉ ra rằng các yếu tố như trình độ học vấn, thu nhập, địa điểm sinh sống và dân tộc ảnh hưởng lớn đến việc tiếp cận các dịch vụ chăm sóc thai kỳ. Ngoài ra, các yếu tố thuộc về cộng đồng như tỷ lệ nghèo đói và trình độ học vấn của phụ nữ trong cộng đồng cũng có tác động đáng kể.

2.1. Yếu tố ảnh hưởng từ cá nhân hộ gia đình đến dịch vụ chăm sóc sức khỏe

Các yếu tố cá nhân như trình độ học vấn, tuổi tác, số lần sinh con, tình trạng hôn nhân và tôn giáo đều ảnh hưởng đến việc sử dụng các dịch vụ chăm sóc sức khỏe. Phụ nữ có trình độ học vấn cao hơn, lớn tuổi hơn và không theo tôn giáo thường có xu hướng khám thai nhiều hơn. Ngược lại, phụ nữ đã ly hôn hoặc sống ly thân, thuộc nhóm dân tộc thiểu số và có thu nhập thấp thường ít khám thai hơn.

2.2. Ảnh hưởng của yếu tố cộng đồng đến lựa chọn địa điểm sinh nỡ của phụ nữ

Môi trường sống và cộng đồng có ảnh hưởng lớn đến quyết định sử dụng dịch vụ y tế. Những phụ nữ sống ở khu vực nông thôn, vùng sâu vùng xa, nơi có tỷ lệ người dân ít học thường ít có điều kiện tiếp cận các dịch vụ chăm sóc sức khỏe thai kỳ chất lượng. Ngược lại, những người sống trong cộng đồng có tỷ lệ phụ nữ sinh con tại các cơ sở y tế cao thường có xu hướng lựa chọn bệnh viện sản hoặc phòng khám sản để sinh nở.

2.3. Sự bất bình đẳng trong tiếp cận dịch vụ chăm sóc sức khỏe theo vùng miền

Sự chênh lệch về kinh tế và cơ sở hạ tầng giữa các vùng miền cũng góp phần tạo ra sự bất bình đẳng trong tiếp cận dịch vụ chăm sóc sức khỏe. Các tỉnh thành phát triển thường có cơ sở y tế hiện đại hơn, đội ngũ y tế có trình độ chuyên môn cao hơn và người dân có thu nhập cao hơn, từ đó có điều kiện sử dụng các dịch vụ y tế chất lượng cao. Ngược lại, các tỉnh miền núi và vùng sâu vùng xa thường gặp nhiều khó khăn hơn trong việc phát triển hệ thống y tế và cải thiện thu nhập của người dân.

III. Giải Pháp Nâng Cao Chất Lượng Chăm Sóc Thai Kỳ Ở Việt Nam

Để cải thiện sức khỏe bà mẹ và giảm tỷ lệ tử vong mẹ, cần có những giải pháp đồng bộ từ cấp quốc gia đến địa phương. Các giải pháp này cần tập trung vào việc nâng cao chất lượng dịch vụ chăm sóc thai kỳ, tăng cường tiếp cận các dịch vụ y tế cho các nhóm đối tượng dễ bị tổn thương, và cải thiện nhận thức của cộng đồng về tầm quan trọng của chăm sóc sức khỏe sinh sản.

3.1. Tăng cường chính sách y tế hỗ trợ chăm sóc sức khỏe cho phụ nữ mang thai

Cần có những chính sách y tế cụ thể và hiệu quả để hỗ trợ phụ nữ mang thai, đặc biệt là những người có hoàn cảnh khó khăn. Các chính sách này có thể bao gồm việc miễn giảm chi phí khám thaisinh nở, cung cấp các gói bảo hiểm y tế phù hợp, và hỗ trợ chi phí đi lại cho phụ nữ ở vùng sâu vùng xa đến các cơ sở y tế.

3.2. Nâng cao trình độ chuyên môn của nhân viên y tế và chất lượng dịch vụ y tế

Việc đào tạo và nâng cao trình độ chuyên môn cho nhân viên y tế, đặc biệt là những người làm việc trong lĩnh vực sản khoa, là vô cùng quan trọng. Cần có những chương trình đào tạo liên tục để cập nhật kiến thức và kỹ năng cho nhân viên y tế, đồng thời đảm bảo rằng các cơ sở y tế có đầy đủ trang thiết bị và thuốc men cần thiết để cung cấp các dịch vụ y tế chất lượng.

3.3. Truyền thông giáo dục sức khỏe về chăm sóc trước sinh và sau sinh

Cần tăng cường công tác truyền thông và giáo dục sức khỏe để nâng cao nhận thức của cộng đồng về tầm quan trọng của chăm sóc sức khỏe sinh sản, chăm sóc sức khỏe trước sinhchăm sóc sức khỏe sau sinh. Các chương trình truyền thông cần được thiết kế phù hợp với từng nhóm đối tượng và sử dụng các kênh truyền thông đa dạng để tiếp cận được nhiều người nhất có thể.

IV. Đánh Giá Lựa Chọn Địa Điểm Sinh Nở Ảnh Hưởng Sức Khỏe Mẹ

Nghiên cứu này sử dụng dữ liệu từ Tổng cục Thống kê, phân tích sâu hơn về cách thức nhiều yếu tố tương tác để định hình lựa chọn địa điểm sinh nở của phụ nữ. Điều này không chỉ đơn thuần là một lựa chọn cá nhân mà còn là kết quả của các yếu tố kinh tế xã hội và văn hóa phức tạp.

4.1. Phân tích hồi quy đa biến đánh giá yếu tố ảnh hưởng lớn nhất

Phân tích hồi quy đa biến giúp xác định yếu tố nào có tác động lớn nhất đến lựa chọn địa điểm sinh nở, từ đó giúp các nhà hoạch định chính sách tập trung nguồn lực vào những lĩnh vực quan trọng nhất. Kết quả nghiên cứu chỉ ra rằng trình độ học vấn, thu nhập và địa điểm sinh sống là những yếu tố quan trọng nhất.

4.2. Nghiên cứu trường hợp điển hình về vùng miền và dân tộc

Nghiên cứu trường hợp điển hình về vùng miền và dân tộc giúp hiểu rõ hơn về những khó khăn và thách thức mà phụ nữ ở những khu vực khác nhau và thuộc các nhóm dân tộc khác nhau gặp phải trong việc tiếp cận các dịch vụ chăm sóc sức khỏe sinh sản. Các nghiên cứu trường hợp có thể cung cấp những thông tin chi tiết và sâu sắc hơn so với các phân tích thống kê thông thường.

V. Kết Luận và Tương Lai Chăm Sóc Sức Khỏe Mẹ ở Việt Nam

Nâng cao chăm sóc sức khỏe mẹViệt Nam đòi hỏi sự phối hợp chặt chẽ giữa các bộ ngành, các tổ chức xã hội và cộng đồng. Việc đầu tư vào hệ thống y tế, cải thiện chất lượng dịch vụ y tế và nâng cao nhận thức của cộng đồng là những yếu tố then chốt để đạt được mục tiêu giảm tỷ lệ tử vong mẹ và cải thiện sức khỏe sinh sản cho phụ nữ.

5.1. Thúc đẩy ứng dụng công nghệ vào chăm sóc sức khỏe thai kỳ từ xa

Ứng dụng công nghệ vào chăm sóc sức khỏe thai kỳ từ xa có thể giúp cải thiện khả năng tiếp cận các dịch vụ y tế cho phụ nữ ở vùng sâu vùng xa và những người có hoàn cảnh khó khăn. Các ứng dụng di động và nền tảng trực tuyến có thể cung cấp thông tin về chăm sóc thai kỳ, tư vấn sức khỏe và theo dõi sức khỏe từ xa.

5.2. Nghiên cứu chuyên sâu về ảnh hưởng của văn hóa và kinh tế

Cần có những nghiên cứu chuyên sâu hơn về ảnh hưởng của văn hóakinh tế đến lựa chọn địa điểm sinh nở và việc sử dụng các dịch vụ chăm sóc sức khỏe sinh sản của phụ nữ. Các nghiên cứu này có thể giúp các nhà hoạch định chính sách thiết kế các chương trình và chính sách phù hợp với đặc điểm văn hóa và kinh tế của từng vùng miền và nhóm dân tộc.

Finally, I would like to express my deep gratitude to my family for their support and endurance when I pursue my postgraduate studies. ii ABSTRACT This thesis research aims to analyze the impact of individual characteristics, household characteristic and communities in utilization of maternal health care services in Vietnam.

Using the latest data of Vietnam’s Multiple Indicator Cluster Survey 2013-2014, it employs the Negative Nominal Model for demand of prenatal care visits and Multinomial Logistic Model for the choice of delivery facility. With respect to the demand of prenatal care visits, the result shows that higher education, higher age, exposure to mass media and no religion increase the number of prenatal care visits while higher birth order, unmarried or separated status, ethnicity group and lower household wealth index decrease the number of prenatal care. Moreover, living in rural, disadvantaged areas and the community with higher illiteracy rate decrease the demand of prenatal care visits while living in the community with higher proportion of women giving birth at health facilities increase the demand. Concerning the choice of delivery facility, more prenatal care visits and exposure to mass media are positively associated with the choice of giving birth at public hospital.

In contrast, suffering the burden of taking care more children, lower household wealth index, living in rural and the community with higher illiteracy ratio adversely affect the choice of public hospital delivery. The results suggest the improvement of maternal health program in rural and underdeveloped areas as well as universal education over the country, especially for the ethnic minority group. World Health Organization (WHO 2014) reported that the global maternal mortality ratio (MMR) in 2013 was 210 maternal deaths per 100 000 live births, decreasing from 380 maternal deaths per 100 000 live births in 1990. However, the ratio in developing regions was 14 times higher than in developed regions. Even though maternal death is generally decreasing worldwide, it has yet to achieve the target of Millennium Development Goal 5 by reducing the MMR by three quarters between 1990 and 2015 (WHO 2014).

The maternal death has direct causes and indirect causes. The direct cause results from arising complications during pregnancy, delivery and postpartum, or improper treatment such as hemorrhage, infection, obstructed labor, unsafe abortion, ectopic pregnancy and anesthesia- related deaths while the indirect cause results from the disease which previously exists or be not due to indirect obstetric causes like hepatitis anemia, malaria, heart disease and tetanus (WHO 2005). It was reported that direct causes made up the higher number of maternal death than indirect causes with 80% of the total MMR (WHO 2005). These complications could be preventable thanks to the intervention of health care such as antenatal care and delivery care, which was introduced by WHO in the safe motherhood package in 1994 (Tran 2012).

Antenatal cares provide the opportunities to pregnancy women and their family to be informed of their health and the growth status of unborn baby. Low birth weights could be prevented if the pregnant women are well acknowledged about their unborn baby’s weight and height during the antenatal care and then improve their diet. In addition, antenatal check-ups detect the danger signs and risks of pregnancy and delivery and make timely interventions. For example, tetanus immunization in the antenatal care period is vital to save the life of the women and their baby.

The management of high blood pressure during pregnancy ensures the maternal health and increase the infant survival (WHO and UNICEF 2003). Furthermore, delivery care also plays an important role in reducing maternal deaths. WHO recommended the child birth at health facility or attended by skilled health staffs to ensure to the safe delivery and give birth to healthy baby. With good hygiene and adequate medical equipment, the delivery at facility could decrease the complications arising from the 1 labor such as hemorrhage, obstructed labor.

In addition, skill health professionals are available in the facilities ensure safe delivery and provide proper emergency management. (Tran 2012) In pursuit of Millennium Development Goal 5 “Improving maternal health”, Vietnam also is making progress in improving the maternal health with the drop of maternal mortality ratio. The World Bank shows that MMR in Vietnam has remarkable improvements in last 15 years in decrease from 81 deaths per 100,000 live births in 2000 to 54 per 100,000 in 2015. The access to antenatal care, an important period for health of pregnant women and their baby and delivery service has also increased.

Multiple indicator cluster survey in 2014 (MICS 5) shows that the percentage of women aged 15-49 with a live birth in the last two years who received antenatal care at least once is 95.8 per cent nationwide. However, there are considerable disparity in maternal mortality ratio and utilization of maternal health care among ethnicity group, place of residence and the regions where the pregnant women are living. MPI 2015 reported that maternal mortality in mountainous areas is more than three times higher than in lowland areas. Furthermore, MICS5 indicates that the times of prenatal care visits differs among the women living rural and urban area, especially regarding having more than 4 visits.

In addition, the ethnic minority groups get more disadvantage of access to maternal health care with 79% of those having 1 visit and 32.7% of those having at least 4 visits compared to 99.1 % of the Kinh group as shown. Therefore, growing disparities in health outcomes and health care utilization have posed a great challenging in recent years. The above challenges lead to several studies in the utilization of maternal health care in Vietnam. Most of them focused on the influence of demographic and socioeconomic factors (Sepheri et al.

2008, Tran et al. 2011, Goland et al. 2012, Malqvist et al. 2012, Malqvist et al.

Demographic factors which were shown to increase the probability of the health services are younger age, low birth order while the factors reported to decrease the probability are separated or unmarried status, unintended pregnancy. In addition, socio-economic factors make greater influence on the use of the maternal health care services. Higher education level of a woman is the most important determinant reported in the previous studies (Sepheri et al. 2008, Tran et al.

2011, Goland et al. 2012, Malqvist et al. 2012, Malqvist et al. Lower household income is also shown to be a strong factor in the likelihood of using maternal health care (Sepheri et al.

2008, Goland et al. Some studies emphasized the disparity in the maternal health care utilization among ethnic majority and minority groups (Malqvist et al. 2012, Malqvist et al. On the other hand, major equity in rural and urban 2 areas also was identified by Tran et al.

(2011) and Sepheri et al. (2008) pointed out the regional disparity in the availability and accessibility to the maternal health care in Vietnam. However, most of them overlooked the community factors, except Sepheri et al. (2008) estimating the impacts of poverty rate.

The omission of the factor may bias the influence on maternity health care utilization, especially for the disadvantaged women. The omission of community effects may result in biased estimates of the roles of factors (Singh et al. Community beliefs and norms encourage or prevent health care seeking behaviors of the women. In addition, community economic development may influence the access to health services and indirectly increase decision making power of women and positive attitudes toward health service use.
